APP: The survey forms are the basis for creating the tasks and reports of your operation in Involves Stage. The next step after configuring the form is to build the questionnaire, defining what information will be collected by the field team, such as price surveys, competitor actions, shelf share, etc. PREREQUISITES: Concept of formulas, Configure form. |
After configuring your form with the data information, objectives and permissions, you must create the form's questionnaire, i.e. the questions that will be answered by the field team. It is in the questionnaire that you define what information you want to pass on or receive from the team, such as price surveys, competitor actions, share of shelf, etc.
The questions are found in the Questionnaire tab, on the Edit Forms page.
On this tab you create the types of surveys that your employees will answer. Upon creating a new form, the page will be empty. To create a new questionnaire, click on the New question button. A pop-up will open on the screen, with specific fields to be filled out:
FIELD NAME
Fill out the name of the question that will appear on the Web.
QUESTION NAME
Fill out the name of the question that will appear on the App. You can use the same title for Field name and Question name, or make them different according to how you want your employee to see the question.
REFERRING TO
The options available for selection in this field reflect those already selected in the Form Theme field. You can create a question referring to the product, brand, product line, supercategory, category or, product-independent, just a simple survey field.
For example, on a price survey form, the theme can be to inform about product lines. Then, you can create questions about the status of the product, price and promotional price - all referring to the product line. Therefore, for each line, the employee should answer a question. If you create a form with 10 product lines, the fields related to those will appear 10 times.
The creation of a question called notes, on the other hand, can be product-independent and therefore appears only once at the end of the form.
FIELD TYPE
In this field, you choose the format of the answer you want to collect or pass on. Below, you will see a detailed description about them. Among the available options are:
Instruction field:
- Descriptive text
- Illustrative image
- Planogram
Answer fields:
- Text
- List of alternatives
- Single choice
- Checkbox
- Whole number
- Decimal number
- Photo
- Date
- Time
- Barcode
The Field type is filled out with one of these 13 formats available for survey answers. . Learn the details of each one below, divided between instruction fields and answer fields. In the instruction fields, you can use:
DESCRIPTIVE TEXT
The descriptive text format does not require the employee to fill out information. It can be used as a phrase or message between form fields.
ILLUSTRATIVE IMAGE
This format allows you to include an image in the form. Just as with Text, it does not require the employee to fill out information. They only see the image.
PLANOGRAM
It allows the registered images to be made available for a supercategory or category, within a specific POS channel. To use this format, the objective of the form is to inform about supercategory or category, and the images previously inserted in the Planogram feature on the Point of sale menu.
In the answer fields, choose between:
TEXT
This field format accepts a variety of characters - letters, numbers and special characters. When you select this option, new fields to fill out appear on the screen. In Number of digits you define the maximum number of characters for the employee´s answer.
LIST OF ALTERNATIVES
This format has a dropdown list to select the option. You should fill out the answer in the Option name list. The Scoring field can be filled in to assign a score to this answer or can be left blank. As you register the answer options, you can reposition them. In addition, the (+) and (-) buttons increase or decrease the fields.
Right underneath, you can select the Define option as the default answer for the question option. Thus, the first answer option registered will be the default answer to the question in the questionnaire. This movement speeds up the answer to the question when the employee is at the point of sale.
SINGLE CHOICE
The single choice format has a feature similar to the alternatives list, by filling out the Option name and Scoring fields and the option to keep the first answer as a standard question.
CHECKBOX
Displays a checkbox for selection. This means that the employee can select it if the situation is true or leave it blank if the situation is not confirmed. For example, in a question about Promotional Price, the employee can indicate whether or not the item being surveyed is at a promotional value at the point of sale.
WHOLE NUMBER
This field for filling out the answer accepts only whole numbers, in other words, without any decimal places. It does not accept periods or commas. In Number of digits you define the maximum number of characters for the employee´s answer.
For example, if you define that the Inventory field can be answered with a maximum of 2 characters, the employee will not be able to answer a value greater than 99, since from the number 100 on there will be 3 or more characters.
DECIMAL NUMBER
This field accepts up to two decimal places. In Number of digits you define the maximum number of characters for the employee´s answer. For example, if you define that the Price field is answered with a maximum of 5 characters - the comma is not counted - the employee can answer values between 0.01 and 999.99. From 1000.00 onwards it would be more than 6 characters.
Note that when you have a question of the Decimal Number type, and it is not related to Other, you will have enabled an additional field. If you want to use this question as one of the four pillars of Perfect Store, all you have to do is choose between Price, Share, Presence and Additional Display Point, finish filling out the rest of the fields and save it.
PHOTO
This format allows you to answer the question with an image. It can be sent directly using the device´s camera or from the photo gallery. In Number of photos you define the maximum number of images for the employee´s answer.
Right underneath you can select the Allow photo to be sent from the photo gallery option. That way, you allow the employee to choose between taking a picture while answering or attaching an image that has already been taken or is in the cell phone´s photo gallery.
DATE
The Date format has a field where it is possible to select the day, month and year.
TIME
The Time format has a field where it is possible to fill out the time in the HH:MM format.
BARCODE
This question format can only be used if the Referring to field is filled out with Product lines or products (SKU).
It allows the employee to use the mobile device's camera as a barcode reader, so it is not possible to enter information manually, only through the camera. The system will check if the bar code entered is the same as that registered in the product lines and products (SKUs).
In Number of digits you define the maximum number of characters for the employee´s answer. The system does not allow you to send a different code than the one previously registered. Learn what types of barcodes Involves Stage recognizes.
Remember that only barcodes already registered in the product and product lines can be validated. And, as well as to check the Validate barcode in the objectives of the form.
CONFIGURE QUESTIONNAIRE
After registering the question in the New question pop-up, you can register a new question by clicking on the Save and Continue button or on the Save button to save the information already registered..
When returning to the Questionnaire tab, you will find a list of question that has already been created.. To edit, simply double click on it. The questions are presented in a row, with the information distributed in columns:
- Sorting
- Field name
- Question name
- Information type
- Objective
You can change the order of the questions by clicking on the question line and dragging it to a new position. In addition, some columns allow a quick configuration about the question, just leave the icon in the color green to activate:
- Mandatory: the answer to the question becomes mandatory and only after it has been answered will it be possible to send the survey;
- Active: activate or deactivate the question on the form;
- Informative: insert the Informative fields data registered;
- Photo report: allows the information collected to be shown on the Photo report.
After configuring the questionnaire, click on the Save button, in the bottom right-hand side of the screen. Done! Your form is ready to use.
ADDITIONAL CONFIGURATIONS
Involves Stage also allows additional configurations to the forms questionnaire. These include the validation between the answer fields, which establishes a range of values when filling out the questionnaire; and also the creation of dependency between the answer fields, where one question generates other questions.
